A infeção crónica pelo vírus da hepatite B (VHB) é um processo dinâmico que reflecte as interações entre o VHB e a resposta imunitária do hospedeiro. Os doentes infectados com o VHB não desenvolvem necessariamente hepatite B crónica. A infeção crónica com antigénio HBe negativo, conhecida na antiga nomenclatura como portador crónico inativo, é uma das cinco fases da história natural da hepatite B. Os 'portadores inactivos' são doentes com doença quiescente que geralmente não desenvolvem complicações. Certos factores são preditivos da perda do HBsAg durante o seguimento. No entanto, a reativação viral (RV), espontânea ou sob tratamento imunossupressor, é possível.
