Foi com Sigmund Freud que as obsessões foram afirmadas como um afeto autónomo e independente, uma neurose, tal como a histeria ou a neurose fóbica. A histeria e a neurose obsessiva, a par da neurose fóbica, permaneceram durante toda a primeira metade do século XX como as grandes categorias de neurose opostas à psicose, uma vez que Freud tinha estabelecido as suas bases estruturais e psicodinâmicas. Tal como a histeria, a neurose obsessiva revela também as grandes questões epistemológicas da invenção por Freud dos grandes conceitos da psicanálise: ego, superego, ansiedade, fantasia, sadismo, masoquismo, oposição entre neurose e psicose, para citar apenas alguns dos conceitos que tiveram um papel decisivo no desenvolvimento da teoria psicanalítica freudiana. A psicanálise das crenças, das fantasias e dos sentimentos de omnipotência narcísica que caracterizam o funcionamento da personalidade obsessiva proporcionou também a Freud a oportunidade de propor uma antropologia psicanalítica (ou antropoanálise) das crenças mágicas e das ideias e comportamentos religiosos.
